Letters For Home: Parent Survey

We all know that parent involvement is crucial to successful early learning, but it can be difficult to find the right language when asking for a parent's help.

This letter helps you cover all the necessary topics to make sure that you're doing all that you can to make the student's school experience more fun and rewarding.

Dear Families,

You and I have the same goal for your child: an enriching year that builds solid skills for reading, writing, and math. But more importantly, you and I both want your youngster to develop self-confidence and a love of learning. To accomplish these goals, I am asking you to help me get to know your child better. Any insights you can share with me will help me tailor my instruction to better serve your young learner. Please use this informal survey to clue me in to what makes your child tick! Answer only those questions for which you are comfortable sharing information with me. Use the back, or extra sheets if necessary. This survey will not become part of your child’s permanent record file, but will only be used by me to learn some of what you already know about your little scholar!

1. How do you describe your child’s personality?

2. Tell me about any special interests, hobbies, or talents your child has.

3. What are your child’s favorite toys or playtime activities?

4. Who are your child’s favorite playmates?

5.How much time does your child spent outside? What are the favorite outside activities?

6. Is your child afraid of anything?

7.Are there other important things you want me to know about your angel-baby?

8.Are there siblings? How old? Do they get along?

9.Are there behavioral, health or allergy issues you’d like me to know about?

Remember, you are your child’s first, best, and most important teacher. Let your child know that you are interested in what is going on at school. Your attitude, your expectation of success, and your support will determine your child’s approach to school. You and I are partners in learning!

Sincerely,

Cindy Middendorf
